Ontario's Diabetes Strategy
As an approach to optimizing diabetes management in Ontario, the government provides funding for diabetes education core teams through an initiative known as The Ontario Diabetes Strategy.
These teams are located in Diabetes Education Centres (DEC) across the province.
DECs are located within outpatient clinics associated with a hospital, within a family health team or are located within the community in community centres. Currently in Ontario (as of 2010) there are a total of 212 DEC’s located within 14 Local Health Integration Networks (LHINs) funded by the Ontario Ministry of Health and Long Term Care (MOHLTC).

- Core member: Trained and certified diabetes registered nurses (RNs) in varying number
- Core member: Trained and certified diabetes registered dietitians (RDs) in varying number
- May include: Social worker, pharmacist, endocrinologists and administrative staff such as program managers, assistants and coordinators depending on the program location
