York Region DEC Program Logic Model
This logic model proposes that if appropriate and skilled staff are employed in the clinic, and if staff training is available
and, if evidence-based practice, literature and protocols are in place, and if a patient referral process is effective,
and if an intake assessment is completed for each client, and if client and family needs are assessed; and
If, as a result of these activities, an individualized education plan is developed and client goals are identified, and if
a case management model is implemented, and if educational materials are available for clients and families, and if
educational sessions are provided based on client and family need, and if care and education is provided based on
cultural requirements; and
If care is coordinated with other programs and specialists, and if ongoing funding is available to support the
operations of the clinic; then
Clients will exhibit improved clinical test values, clients and families will report satisfaction with the program and
improved self-care knowledge and lifestyle choices, and clients will report improved diabetes self-management and
control, and improved quality of life; and
Clients will exhibit ongoing improvement in self-care practices, and prevention of further diabetic complications,
and clients will be empowered in the management of their care, and clients will have increased awareness of risk
factors of, and strategies to manage, diabetes; and
Healthcare providers will have increased knowledge of diabetes management through dissemination of program
results and health policies will be developed to improve quality of access to diabetes care.
