Goals of the York Central Hosptial Diabetes Education Centre Program
The York Central Hospital DEC team has set goals for the clinic based on the Ontario Diabetes Strategy. The team has recognized that it aims to accomplish the following with it's clientele:
- Provide on-going education and counselling to adults with type 2 diabetes and their families regarding managing diabetes to increase awareness and behaviour change for healthy living
- Develop and implement diabetes educational tools and resources for adults with type 2 diabetes including populations where English is the second language
- Empower clients to be self-directed in learning the skills and behaviours needed for successful diabetes self-management
- Develop and implement health policies that improve the quality of and access to diabetes care
- Establish linkages with community partners that also focus on diabetes care, education and management
Program Participant Outcome Objectives
Through the attainment of the Program Goals, it is the belief and expectation of the DEC Team that clients with type 2 diabetes who participate in the DEC Program will:
- Increase knowledge and understanding of:
- Diabetes and complications to other organs as a chronic lifelong condition
- Blood work and parameters of normal values
- Complications related to elevated blood glucose levels
- Illness and its effect on diabetes self care
- Diet, medication and insulin management
2. Increase the frequency of necessary behaviour related to:
- Self monitoring of blood glucose levels with their blood glucose monitoring device
- Adjustment of insulin doses based on blood glucose levels & amount of carbohydrates taken with meal
- Maintain blood glucose levels within acceptable parameters for age
- Reducing HbA1C and blood pressure values based on their own personal health goals
- Improving values in their lipid panel
- Frequency of exercise
- Follow a healthy eating plan
