Diabetes Clinic Evaluation Recommendations
It is the expectation that the Diabetes Education Clinic (DEC) program evaluation will examine the program's effectiveness in providing support and education in culturally sensitive environments in diabetes self-management for clients and their families. The ultimate goals of the DEC are to meet the targets specified by the Ontario Diabetes Strategy. Included in the evaluation is the consideration of the challenges the team have experienced related to use of evaluation findings. With this evaluation, findings are to be considered within the utilization focused framework to foster and ensure stakeholder engagement. After development of the program logic model and data collection matrix, in a Utilization -Focused theoretical framework, the evaluation team explored the program and its goals and offer some of the following recommendations:
Suggested Recommendations
1. Was funding adequate to support the interdisciplinary team to meet the educational needs of the diverse cultural
client population ?
Recommendation: Funding to continue being distributed to clinics based on the educational requirements that are
specific to the diverse population served. Funding resource utilization to be utilized for maximum
impact on client outcomes and with specific attention to cultural needs of clients and families.
2. Does the interdisciplinary team work well and efficiently together to achieve clinic's goals ?
Recommendation: Review staff mix, model and ratio based on meeting educational needs of clients based on standard
of care at other clinics of comparable service.
3. Are clients satisfied with educational instruction received at the clinic?
Recommendation: Continue monitoring clients' perspectives on education received and review time-lines of clients'
achievement of goals.
4. Do clients and their families express comfort and independence with diabetes care while at home?
Recommendation: Establish a post test review of core competencies with follow up.
5. Do clients return for further education with concerns regarding the instruction that has been provided?
Recommendation: Staff to follow up with clients who are struggling with core skill competences in relation to their
diabetes self -management.
6. Do clients and their families retain a level of independence with education provided?
Recommendation: It would be beneficial to devise a communication follow up with clients' primary care providers to
assess for complications and or changes in blood glucose with clients discharged from the clinic.
